St. Jude Heritage Medical Group is actively seeking a seasoned Geriatric Nurse Practitioner to become a vital member of our dedicated Skilled Nursing Facility team. This dynamic position involves overseeing patient care across 3-5 skilled nursing facilities within the Northern Orange County region. Team collaboration is paramount, with Geriatricians and Nurse Practitioners functioning as peers in a cohesive environment that extends to all clinic staff. Ideal candidates will excel at independently managing their portion of the team's patient panel, while also thriving within a collaborative team environment. This full-time role includes a comprehensive schedule, covering in-person visits from Monday to Friday and on-call duties over the weekend, ensuring continuity of care for our patients. • Full Time Position
• Requires at least 5 years of NP experience in adult or geriatric medicine
• 7 days a week (in person M-F, call includes Saturday (in person) and Sunday (by phone)
• Outpatient Care
• Compensation is between $150,577 and $194,793 per year
• The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, call stipends, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits.
